Using the Work Order Request Form:

These instructions will take you through the process of filling out each of the fields on the Work Order Request Form one by one. They must be completed properly for your work order request to be fulfilled. Please take the time to read through them before moving on to the form.
1. Date: Please enter today’s date, the date that you are actually submitting the request. DO NOT enter any other date, such as the date that the problem actually started occurring. The purpose of this field is not to tell us how long you’ve been having the problem, but to tell us when we received this request.
2. Name: Please enter your name only (the name of the person actually filling out this form.) The only exception is that if you’re filling this form out for a department head (Director, Chair, etc.) or a building manager at their request you may enter their name, title and phone number if they wish to be contacted directly. In all other cases, you need to enter your own name, title, and phone number in the required fields.
3. Title: This field is to be filled out with the position title of the person whose name is listed in the "Name" field above. No exceptions.
4. Department: Please enter the name of the department which is making the request (the department to which the requestor is assigned.)
5. Account #: Please enter the account number to be charged for any chargeable requests.  Putting an account number in this field does not neccessarily mean that the department will be charged.  Again, you must be authorized to sign on the account listed to submit this form with any account number.
6. Phone: The phone number of the person listed in the "Name" field.
7. E-mail Address: The e-mail address of the person listed in the "Name" field. List the entire e-mail address, not just the name.  (ie: yourname@ufl.edu, not just dandrews)
8. Building: Scroll down the list and select the building that the request is for. If you work in one building and the work requested is to be done in another, list the building where the work will be done.
9. Floor: List the floor that the request is for. If the request is for an outdoor site simply type "outside" in the field.
10. Room: Every room in every building on campus has a room number assigned to it, even hallways closets, and restrooms. That number MUST be placed in this field. This is not optional. Please do not use phrases such as "in the director’s office" or "the restroom near accounting", etc. This field is to have only the room’s number. .
11. Request: This is where you can type in your request. If you would like, you can mention what kind of room the request is for. This is also where you can be very specific as to what you would like done. The more specific you are, the better. Knowing exactly what the situation is before going will help us to reduce the amount of time and number of trips required to finish the job. This will save the university money and your department’s request will be fulfilled more quickly and efficiently.
